New and Existing Patient Forms:
We want to welcome you to our office! Below are the necessary registration forms that must be completed prior to your appointment in black or blue ink, NOT in pencil. Please bring these completed forms with you to your appointment. Also remember to bring your insurance cards, your insurance mandated referral (if required), and photo identification.
If your insurance company requires a co-payment, this is expected at the time of your appointment. Every new patient is allocated one full hour of the physician's time. It is our policy to charge a $25.00 fee for not showing up for your appointment or appointments not cancelled at least one business day prior to the scheduled time. This is the patient's responsibility and we will not bill your insurance for this charge.
